    Community First and its partners are surveying Hawai‘i Island residents throughout the month of November to better understand what communities need to improve their health. The survey is part of Access to Care, a collaborative working to transform healthcare on Hawaii Island.

    You have a chance to win $250 grocery store gift cards, drawn weekly!  Find entry information at www.accesstocarehawaii.org, where you’ll also find the short, anonymous survey, or look for Access to Care at one of the island-wide pop-up events at participating grocery stores and farmers markets throughout the month of November.

    The results of the Access to Care survey, once final, will be shared with policymakers, healthcare systems and social service providers so they can work together to streamline services, identify gaps in care and effect real change across the island.
